Integrated care pathway

Integrated care pathways are guidelines given to professionals, which are based on national treatment guidelines and applied to local conditions. The purpose of integrated care pathways is to ensure the patient receives appropriate and fluent treatment across treatment units, organizations and different treatment professionals. In addition, their goal is to unify treatment practices and clarify the gradation of research and treatment.

The introduction of digital integrated care pathways is currently being developed in the Eastern collaborative area. Digital integrated care pathways provide a clear picture of each stage of an individual’s care pathway, and enables the patient to better take part in the planning and follow-up of their own treatment.
